Good morning! You can log into your account and download any paperwork pertaining to your visit. You can also contact customer support and they would be happy to assist you.

I paid extra pointlessly, then they lied about refunding me.
I paid extra for them to do my state registration for me. They emailed back that they needed another form of residency proof when they did not, in addition to other unnecessary information. I completed the process myself quickly and easily with the materials they already had. Never got my money back for the extra "service", even after they supposedly refunded the fee for it as they informed me in their smug copy/paste response to this review. I did get my card quickly.

In reviewing your account, our team requested an additional proof of residency because the address on the identification provided did not match the residency documentation submitted. In these situations, we are required to verify residency before we can complete the state registration process on a patient's behalf.
We understand that you were able to complete the state application yourself, and we're glad you received your card quickly. As a courtesy, we have refunded the additional service fee associated with the state registration assistance.
We appreciate your feedback and are always looking for ways to improve our processes and communication with patients.

Elevate helps take the guess work out of getting a license. They can even do the application for the Arkansas card.
